AOC Referral Process

Patients can take this referral form to their family physician or walk-in physician who can fax this directly to our office.  Our referral forms are readily available on MedAccess and Accuro EMR. Our office will be in contact with you directly for your initial appointment.

All appointments provided by our program are covered by 

Alberta Health Care Insurance Plan (AHCIP).

All appointments will be conducted via telephone.

Patients are seen within approximately 4 weeks of receiving referral.
